Job Summary
The Social Work Supervisor plays a critical role in overseeing and guiding a team of social workers in providing high-quality services. This position involves ensuring compliance with relevant regulations, providing comfort, support, and dignity to those nearing the end of life. The ideal candidate will possess strong leadership skills, and extensive knowledge of social work practices.
Duties:
- Oversee the development and implementation of treatment plans based on patient assessments including: caregiver abilities or lack thereof
- Assessment of psychosocial status of patients/families/caregivers related to the patient's terminal illness and communicate findings.
- Counsel patient/ family/caregivers as needed
- Ensure compliance with HIPAA regulations and maintain accurate medical documentation for all client interactions.
- Provide training and mentorship to social work staff on applied behavior analysis and effective counseling strategies.
- Collaborate with interdisciplinary teams to coordinate care for patients.
- Monitor and evaluate the effectiveness of programs and interventions, making adjustments as necessary to improve outcomes.
- Facilitate regular team meetings to discuss case management, and promote professional development.
- Link to appropriate community resources

Qualifications
- Must have a master’s degree in social work
- Must have minimum of two (2) years of documented supervised experience in health care, hospice experience a plus
- Must understand hospice philosophy
- Must understand issues of death/dying.
- Must have the ability to travel throughout the Minden/Shreveport/Bossier and surrounding areas.
- Strong skills in patient assessment, clinical documentation, and adherence to HIPAA standards.
- Excellent communication, leadership, and interpersonal skills to effectively manage a diverse team.
- Ability to work collaboratively within an interdisciplinary team environment.
Job Types: Contract, PRN
Pay: $25.42 - $26.93 per hour
