Hands from the Heart Home Health Care Services is actively seeking an experienced Intake Coordinator/Marketing Assistant to join our thriving team.

What role will you play in the success of the organization:
- Facilitate onboarding of new clients into the agency
- Coordinate staffing of cases between HR, Marketing and Client Services
- Communicate with Service coordinators and Referral Sources
- Request authorizations
- Confirm schedules with Caregivers and Clients
- Assist with communications for potential clients
- Monitor: statistics of marketing phone calls, transfers/new services
- Follow up with transferring cases
- Maintain well organized case notes
- Attend follow-up meeting and meet and greets with consumers if needed.
- Initiate, monitor and provide ongoing oversight of consumer service plans and eligibility requirements and individual progress in Medicaid Home and Community Based Waiver Programs to ensure timely and effective services for consumers.
- Advocate for and represent consumers who request support and for those who are unable or incapacitated to advocate for themselves.
- Complete all assigned documentation a timely manner.
- Other work-related duties as assigned.
Qualifications:
- Bilingual Spanish and English Speaking Preferred
- Must have at least 1 full year of relevant experience
- Must have exceptional interpersonal skills and be able to deal with differing personalities daily.
- Must have ability to interact with people on all levels of the organization in a professional manner.
- Must have effective and professional communication skills
- Must have proven organizational skills and independently prioritize.
- Consumer Interaction: must be able to maintain positive interactions with consumers, their families, and attendants.
- Demonstrates the ability to utilize a wide range of appropriate knowledge, skills, and abilities to achieve desired outcomes within the scope of the job responsibilities.
