Hands of God Hospice is a privately owned hospice company serving Sangamon, Macon, Christian, Moultrie, Logan, and Shelby Counties. Hands of God Hospice is a Patient-Centered/God-Focused Agency that is called to "be the hands and feet of God" We believe that hospice is a ministry that simply loves others, whether it be patients, caregivers, coworkers, referral sources, and community. We are looking for an ideal candidate that appreciates our values and preferrably has hospice and/or critical thinking nursing experience. The position is largely Monday-Friday with the exception of the occasional weekend on-call (paid) coverage and on-call (paid) rotating during the week.
The ideal candidate will have a strong background in end-of-life patient care and desires to serve God as well as serving our patients.
**Job Overview:** We are seeking a compassionate and dedicated Hospice Nurse to join our team. The Hospice Nurse will provide end-of-life care and support to terminally ill patients and their families. Communication and integration with the families and facilities where our patients live is an expectation. We believe that it is impossible to over-communicate with our agency.
This role requires a high level of empathy, medical expertise, and the ability to provide comfort during challenging times.
**Duties:** - Conduct patient assessments and develop individualized care plans - Administer medications and treatments as prescribed - Monitor patient symptoms and condition, providing pain management and symptom control - Collaborate with interdisciplinary team members to ensure holistic care - Educate patients and families on disease process, treatment options, and end-of-life care - Maintain accurate medical documentation and ensure compliance with HIPAA regulations . Timely documentation is a must. Reliable transportation is also mandated.
**Qualifications:** - Registered Nurse (RN) license in the state of Illinois - Previous experience in hospice or palliative care is preferred - Strong knowledge of anatomy, physiology, and medical terminology - Ability to provide compassionate patient care in a sensitive environment - Proficiency in medical documentation and electronic health records - Excellent communication skills and ability to work effectively in a team setting
