What are the responsibilities and job description for the Waiver Case Manager position at Handy Help LLC?
We are seeking a Waiver Case Manager to join our dynamic team of case managers, providing case management services for individuals receiving wavier type CADI, BI, DD, VA/DD in Ramsey County. The case manager will work directly with the Program Supervisor and may be called upon to assume direct care role as needed.
The mission of Handy Help is to offer culturally and linguistically appropriate services in a person-centered manner to those we serve. We believe that individuals with MI/CD and co-occurring disorders possess unique strengths, interests, aspirations, and abilities.
Handy Help has a diverse and experienced leadership team that oversees and supports a varied staff of mental health practitioners and social workers. This approach makes our services easily accessible, particularly for clients with limited English proficiency who may prefer to engage with a mental health professional sharing a similar cultural background, instead of relying heavily on interpreters.

Qualifications
REQUIRED, Bachelors Degree in Human Services or related field.
· Strong written and verbal communication skills, computer skills, and the ability to complete paperwork accurately and in a timely manner
· Excellent organizational and time management skills.
· Ability to work both independently and within a team.
· Quick learner.
· Successfully clear DHS background check.

Responsibilities
· Communicate with community professionals and other service providers to ensure that services for client are coordinated
· Positively represent agency, program and participant needs, both internally and externally
· Assure that documentation meets billing requirements per contract
· Approve direct client assistance requests, and track contract spending
· Maintain outcome-based statistics, demographics, and reporting in keeping with expectations of funder and HandyHelp requirements
· Coordinate integrated services and supports necessary to assist each client as they work towards their goal of recovery
· Employ a holistic and culturally-appropriate approach when interacting with clients including promoting health and wellness
· Partner with clients to develop an individualized care plan that identifies services and supports that will enable each client to live as independently as possible.
· Treat all clients with dignity and respect, providing services that are respectful of the person’s culture, age, diagnosis, gender, sexual orientation, religion
· Complete required paperwork including case note
· Participate in professional meetings and trainings
· Comply with HIPPA and Data Privacy Practices
· Actively participate in agency’s undoing racism plan and demonstrate an ability to relate to and advocate for diverse client needs and interests
Pay & Benefits:
· Salary: $ 52,000-$65,000 DOE
