The Community Health Worker at Hanna Z Social Day Program plays a crucial role in enhancing the health and well-being of our adult participants. This position involves facilitating access to healthcare services, and supporting individuals in managing their health within a community-based setting.
KEY RESPONSIBILITIES
- Completes initial assessment for new, eligible participants with a particular focus on high risk, vulnerable populations. Establishes and maintains trust with program participants. Provides referrals and linkage to services as well as support and advocacy for participants.
- Work closely with healthcare providers, social workers, and other staff to develop and implement individualized care plans.
- Collect and maintain accurate records of participant interactions, health outcomes, and services provided.
- Knowledge of community resources and the ability to connect participants to appropriate services.
- Participate in team meetings and receive feedback to improve skills and techniques.
- Perform other duties as assigned by the Program Director to support the overall operation of the program.
- Maintain a positive attitude and work collaboratively with all center staff.
Requirements:
- Must be 18 years of age and above High School diploma or equivalent. Associates Degree in Human Services or completion of a recognized Community Health Worker training program preferred.
- Proficiency in Microsoft Office products, Google Suite and ability to work various software applications preferred.
- Must provide a health examination signed by a physician, TB clearance, and current CPR/First Aid Certification or obtain within six months of employment.
- Flexibility with working hours, including weekends and evenings, if necessary.
Preferred Qualifications:
· At least one year of experience in health education, community outreach, or a related field. Experience working with adult populations in a community setting is advantageous.
