Position Requirements:
- Full time, typically Monday through Friday, 8am-4:30pm with flexibility and potential for on-call rotation
- Must be licensed by state of New Hampshire as a LPN or RN
- Minimum of (3) years experience in a healthcare setting
- Demonstrate ability to accurately assess and use independent clinical judgement for all SNF/LTC referrals
- Knowledge of Medicare, Medicaid, and Managed care billing practices and level(s) of care guidelines
- Computer proficiency; ability to effectively access and utilize various computerized systems
- Strong and effective English communication skills
- Ability to operate in a high paced environment while maintaining a positive supportive presence to all customers
- Must have reliable personal vehicle daily for hospital and community based referrals
- Supports and positively contributes to the workplace culture and values of HHHCC
Duties:
- Assess all inquiries within a timely manner, facilitate admission decision and ensure a positive experience for all parties involved
- Complete comprehensive clinical, financial, and psychosocial assessment to identify appropriate placement; and ensure patient/resident needs are met
- Ensure proper completion and distribution of required paperwork; including communication to all department heads
- Stay abreast of and ensure compliance with all admission, discharge and pertinent regulations
- Conduct facility tours; manage applications and sales process
- Responsible for case management of all managed care patients; facilitate updates as required, serve as liaison for communication with insurance agency, provide updates to patient/family, assist in the discharge planning process and maintain individual files.
- Meet census goals by working with interdisciplinary and consultant team for deliverables; report to Quality Assurance
- Develops and works to nurture and maintain referral relationships through excellent customer service oriented process
- Identifies and participates in marketing opportunities to maximize referral base; and enhance relationships with physicians, social workers, discharge planners, case managers and others that are a valued customer in the admissions process.
- Assists in the activities of the nursing department to ensure facility standards of care and state and federal regulatory compliance is being met
- Participate in facility Care Transitions meetings focused on reducing avoidable hospital readmissions and ensuring a smooth transition of care amongst healthcare institutions.
- Meets with resident and family on day of admission to welcome them; provide follow-up visits to ensure continued customer satisfaction.
- Collaborate with Social Services and assist in completion of Admission Agreement , discharge planning, bed holds, room transfers or other duties as requested.
- Foster teamwork through assisting in the nursing department as time allows. Examples include but not limited to facilitation of admission process, chart set-up, transcription of orders/verification, and medication administration.
