Hanover Hill Health Care Center a Skilled Nursing and Long Term Care Facility in Manchester, NH is now hiring for the position of Social Worker. We are seeking a motivated, qualified Social Worker with excellent leadership skills.
The primary purpose of this job position is to assist in the planning, organizing, developing, of the Social Services Department to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
Education and Experience Requirements:
- Must possess a minimum of a bachelor’s degree in social work or a related field from an accredited institution.
- Minimum of one (1) year of experience in a social service setting, preferably in a healthcare or long-term care environment.
Major Duties and Responsibilities:
- Assist the Director of Social Services with facility social service programs.
- Collaborate with the community to address facility and resident needs.
- Participate in discharge planning, resident assessments, and care plan development.
- Provide referrals to external agencies for services not available within the facility.
- Educate residents and families about available financial assistance programs (e.g., Medicare, Medicaid).
- Ensure that the Social Services department adheres to federal, state, and local regulations.
- Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility.
- Chart all progress notes and ensure they are informative and descriptive of the services provided and the residents’ response to services.
- Coordinate and work with other departments on social service activities and programs to collaboratively meet the needs and resolve resident issues.
- Interview resident/families as necessary and in a private setting, to obtain social history.
- Involve the resident/family in planning social service programs when possible.
- Assist in arranging transportation to other facilities when necessary.
- Work with the facility's consultants a necessary and implement recommended changes as required.
- Maintain excellent working relationships and collaborate with other departments and agencies to maintain a positive working environment.
- Manage and document any resident complaints or grievances, reporting actions taken to resolve issues in accordance with regulatory policy.
- Assist in maintaining a written record of the residents’ complaints and/or grievances.
- Assist in maintaining and coordinating a Resident Council.
- Maintain confidentiality of all patient information to assure patient rights are protected.
