Job Description :
- Provide care and assistance to residents in a Skilled Nursing Facility (SNF) setting.
- Ensure patient safety and comfort throughout the shift.
- Assist with activities of daily living, such as bathing, dressing, feeding, and mobility.
- Document care activities using the Point Click Care charting system.
- Collaborate with nurses and other healthcare professionals to deliver high-quality care.
- Maintain a clean and organized work environment.
- Follow all facility policies and procedures, as well as state and federal regulations.
- Respond promptly to patient calls and requests.
- Support patients' emotional and psychological needs.
- Participate in team meetings and ongoing training as required.
