Coordination with field staff including: -Daily report from nurses via system message -Followup on out of parameter/unusual issues, followup to make sure anything reported to you has an order, comm note, infection control, fall report written if appropriate -Assisting with ordering DME supplies for patients and followup until completed -Assist field staff with coordinating with MD and sending communication notes to MD offices when needed -Ensure all lab needs are scheduled and orders written, Review lab results upon receipt and send to MD. -Anti-coagulation therapy monitoring including: scheduling PT/INR's, reporting results to MD, document response/orders for next PT/INR -Make sure staff gets DC notices signed at least the week prior to recert/dc visit and documented in note

Documenting Reviewing/QA tasks including: -Review SN notes and Pass when all corrections made by SN - -Ensure coinciding orders, fall reports, Post ER communications and other communications are completed and accurate. Ensure medications list are up to date. - Monitor wound care patients for progress, weekly measurements, change in status and assist with obtaining orders for change in wound care as needed. -Ensure wound care orders are appropriate and documented correctly

Care coordinating assistance including: -Contact each monthly B12 patient the week prior to ensure medication in home, Ensure all B12 orders are correct. - Ensure adequate supplies are in house for FC/Ostomy patients and that they are scheduled appropriately each cert period. -Monitor CHF patients for weight fluctuations, and call CHF patients that are only seen every other week by SN, on the opposite weeks for tele-care monitoring of CHF. -Contact patients with re-certifications scheduled, the week prior to ensure coordination for recert visit if needed.

Billing Assistance including: -Ensuring notes are reviewed in a timely manner needed for billing -Entering First billable Dates when ready -Requesting notes/orders/paperwork from staff to complete billing needs.

Other Tasks including but not limited to: -Field visits if indicated (all field staff and Case Manager are full and patient's still need to be seen) -Assist in field staff training

Alternating weekend on call

Job Type: Full-time

Pay: $20.00 - $24.00 per hour
