What are the responsibilities and job description for the Social Worker position at Happy Life Adult Day Health?
POSITION SUMMARY: This position is responsible for planning and administering social service programs within the facility. He/she assists in developing Happy Life Adult Day Health Center policies regarding participation in community planning for health and welfare services. The Social Worker plans and assists in coordinating coordinates or assisting with the discharge planning and coordination of community resources.
RESPONSIBILITIES/ACCOUNTABILITIES
- 1 Assists with planning and implementing a comprehensive social services program which provides for counseling and other support services for Participants and families;
- 2 Identifies medical-related social needs of Participants, provides appropriate services to meet the individual, as well as collective needs of Participants, and maintains records relating to the Participants’ social work needs and care;
- 3 Consults with other department heads regarding interdisciplinary issues, as well as maintenance of appropriate records;
- 4 Works directly with Participants and families experiencing personal and environmental difficulties or concerns related to the Participant's physical or emotional condition;
- 5 Promotes the preservation of the Participant's physical and mental health and to prevent the occurrence or progression of personal and social problems;
- 6 Maintains a written record of the frequency and nature of the social service consultation and services provided or obtained;
- 7 Evaluates each Participant's social needs then formulates the plan for providing care and records the plan in the Participant's medical record. Periodically re-evaluates in conjunction with the Participant's total plan of care;
- 8 Plans and implements family meetings to provide a forum for ongoing discussions between participant/family and treatment team with a primary focus of discharge planning and or home support and services;
- 9 Collects pertinent social data upon admission of each Participant and places it in the medical record, including information about the personal and family problems related to the Participant's illness and care, support network, actions taken to meet the Participant's individual needs and eventual discharge to an appropriate level of care based on functional capacity. Pertinent social data shall be made available to the attending physician and other appropriate staff members;
- 10 Acts as a liaison between Participants, families, outside agencies, and the facility Administrator to ensure that the Participant's rights are maintained;
- 11 Functions in a manner that adheres to all policies/procedures of the facility, as a representative of the Happy Life Adult Health administration;
- 12 Documents and prepares any reports requested by the administration; also councils and provides assistance when change in financial status occurs for Participants in the Happy Life Adult Day Health
- 13 Coordinates discharge planning and assists with the creation of an organized discharge plan on all Participants;
- 14 Concerns his/herself with the safety of all facility Participants in order to minimize the potential for fire and accidents. Also, ensures that the facility adheres to legal, safety, health, fire and sanitation codes by being familiar with his/her
role in carrying out the facilities fire, safety and disaster plans;
- 15 Ensures that Participants and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights;
- 16 Performs other duties as requested.
SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
- 1 Certified by a licensed physician for employment to be in good health including freedom from communicable disease, including tuberculosis in the communicable form, according to our employee health policies.
- 2 Education / Experience Baccalaureate degree in social work or sociology or psychology with experience in a licensed health care facility (preferably a geriatric care facility). CSW or LCSW credential degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, Sociology, Special Education, Rehabilitation Counseling and Psychology. Qualifications. The social worker must have at a minimum a bachelor's degree in human services from an accredited college or university and at least one year's recent experience working with adults in a professional capacity.
- 3 Knowledge of assessment, care planning and interventions is preferred.

- 2 Carries out other tasks as requested in situations where hands-on intervention/participation may be required.

Pay: $30.00 - $38.00 per hour
