Harbor is seeking a Children's and Outreach Coordinator for our Chrysalis program! The Coordinator of Chrysalis Children’s and Outreach actively identifies potential referral sources and recruits women who meet the level of care and would benefit from the SUD Residential Treatment. The Coordinator would provide community presentations regarding the dangers of drug use during pregnancy to vulnerable populations and others who interact with vulnerable populations.

Position is full-time, 1st shift, 9AM to 5PM.

Education/Experience/Other Requirements:   

  • Requires an Bachelor’s Degree in Healthcare Administration, Business, Psychology or comparable degree. Comparable experience with an Associate’s degree in lieu of Bachelors.
  • State of Ohio clinical licensure preferred (LSW, LPC, LISW, LPCC, LCDC or LICDC).
  • Experience in a business development, case management, and administrative support preferred.  
  • Experience with private and public insurance, and an understanding of the business of mental health preferred.
  • Knowledge of 12-step groups of recovery. Experience working with individuals with substance use disorders or women and children preferred.
  • Able to work successfully with interdisciplinary teams.
  • Must successfully demonstrate the basic competencies listed above, as determined by your supervisor’s review, within 90 days of hire.
  • When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.

Essential Job Competencies/Primary Duties:

  1. Identifies potential referral sources and provides outreach and education about services available.
  2. Provides information to potential participants about the program to help better understand program structure, rules and supports available to increase likelihood that the individual is complete the program.
  3. Remains current with drug use trends and the dangers of drug use during pregnancy.
  4. Conducts community presentations regarding the dangers of drug use during pregnancy.
  5. Facilitates admissions and intake documentation.
  6. Responsible for the quality and completeness of the clinical record.
  7. Facilitates individual and group educational programming for residents and their families and linkage to services, or transportation, as needed.
  8. Participates in treatment team meetings.
  9. Assists with discharge planning as needed and post-discharge follow-up.
  10. Completes grant tracking and records information needed for grant or other outcomes reporting.
  11. Assures children of enrolled clients are screened and receive care from an appropriate provider when necessary.
  12. Facilitate appropriate children’s medical appointments and transportation for mothers and children in the program.
  13. Facilitate with enrolled mothers the appropriate coordination of care and visitation with themselves, the children, other care givers and/or the appropriate county Children Services agency.
