Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities. Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants. We are currently seeking an extremely talented Care Coordination Assistant to join our team at the Elder Service Plan in Mattapan.

Role :
The Elder Service Plan (ESP) Care Coordination Assistant works closely with the site Care Coordinator(s) to perform essential administrative and scheduling support for ESP participants. The Care Coordination Assistant proactively ensures medical and psychiatry appointments are scheduled, including all needs around transportation and other services. In addition, the Care Coordination Assistant supports teams by performing CMS-regulated / Medicare administrative tasks including management of all aspects of Care Plans.

Responsibilities :
Scheduling and Appointment Coordination
- Ensures regulated visits and physicals are scheduled in accordance with the Elder Service Plan / PACE regulations.
- Schedules follow up and acute visits as needed.
- Uses reporting tools, Excel files, and Electronic Health Record (EHR) to perform tasks efficiently.
- Supports new enrollee experience by providing scheduling and coordination for initial assessments.
- Ensures initial and follow up psychiatry appointments with ESP providers are scheduled and communicated to participants.
- Arranges transportation and supportive services when needed for participants.
- Engages families / support network as needed; able to identify who is most appropriate to contact based on participant’s medical record.
- Ensures appropriate appointment confirmation reaches participant and / or their relevant contacts, including Nursing Homes / Assisted Living Facilities.
- Supports community-based appointment coordination (e.g., scheduling, appointment maintenance, confirmation, etc.).
