Harbor Health  operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly, a program that allows frail elders to live in their homes and communities.  Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants. We are currently seeking an extremely talented Care Coordination Assistant to join our team at the Elder Service Plan in Mattapan.

Role :

The Elder Service Plan (ESP) Care Coordination Assistant works closely with the site Care Coordinator(s) to perform essential administrative and scheduling support for ESP participants. The Care Coordination Assistant proactively ensures medical and psychiatry appointments are scheduled, including all needs around transportation and other services. In addition, the Care Coordination Assistant supports teams by performing CMS-regulated / Medicare administrative tasks including management of all aspects of Care Plans.

Our ideal candidate would possess excellent customer service skills, an ability to be organized with a high volume of tasks, a capacity to perform detailed workflows consistently, and an openness to supporting teammates in their roles. .

Responsibilities :

Scheduling and Appointment Coordination

  • Ensures regulated visits and physicals are scheduled in accordance with the Elder Service Plan / PACE regulations.
  • Schedules follow up and acute visits as needed.
  • Uses reporting tools, Excel files, and Electronic Health Record (EHR) to perform tasks efficiently.
  • Supports new enrollee experience by providing scheduling and coordination for initial assessments.
  • Ensures initial and follow up psychiatry appointments with ESP providers are scheduled and communicated to participants.
  • Arranges transportation and supportive services when needed for participants.
  • Engages families / support network as needed; able to identify who is most appropriate to contact based on participant’s medical record.
  • Ensures appropriate appointment confirmation reaches participant and / or their relevant contacts, including Nursing Homes / Assisted Living Facilities.
  • Supports community-based appointment coordination (e.g., scheduling, appointment maintenance, confirmation, etc.).

Care Plan and Site Support

  • Books appointments and ensures each participant has a Care Plan scheduled every 6 months.
  • Ensures notification letters are sent in a timely way.
  • Makes appointment confirmation / reminder calls as needed.
  • Facilitates care plan sessions including ensures adherence to time constraints, dials in families and participants, introduces staff, keeps time, guides discussion, and documents attendance in EHR.
  • Edits typos, prints, and mails completed care plan, medication list and attendance sheet following care plan meetings.
  • Takes notes for Interdisciplinary Team meetings and escalates events requiring tracking per CMS regulation (e.g., falls with injury, pressure ulcers).
  • Provides coverage for Care Coordinators during planned and unplanned absences.
  • Ability to collaborate effectively with other members of the team.
  • Requirements :

  • High School diploma or GED with two years of experience working in a health care environment, or Associates degree with one year of healthcare experience, or Bachelor’s degree in related field.
  • Experience in customer service (e.g., scheduling, call center, front desk) preferred.
  • Excellent communication skills with ability to communicate with varied individuals including participants, staff, and supervisor – both one-on-one and in group setting.
  • Must have a desire and ability to communicate effectively with geriatric population.
  • Demonstrated ability to learn and consistently adhere to technology workflows.
  • Demonstrated attention to detail and critical thinking skills.
  • Demonstrated ability to develop relationships with participants, families and clinical staff.
  • Demonstrated competency with technology, including typing skills and software (e.g., Electronic Medical Records, Microsoft Office 365 including Word, Excel, Outlook and Teams)
  • Must have access to reliable internet as needed
  • Must be able to work efficiently from a remote location as needed and / or travel between Harbor sites in a timely manner
