What are the responsibilities and job description for the Staff Nurse Per Diem position at Harbor Health Services, Inc.?
Harbor Health is always seeking Staff Nurses (RN) to join our per diem pools at our to cover open shifts at our community health center locations including Boston, South Shore and Cape Cod. We have day shifts, some evenings until 8:00 PM and Saturday shifts.
Our health center locations include:
- Daniel Driscoll-Neponset Health Center and Geiger Gibson Community Health Center in Dorchester,
- Harbor Community Health Center-Plymouth and
- Harbor Community Health Center-Hyannis.
Role:
The Staff Nurse provides nursing services at the top of his/her license to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc. Duties include but are not limited to: triage; access; case management; and nursing support of clinical sessions.
Responsibilities include:
- Assists providers and Care Team members in caring for patients.
- Performs routine nursing care to patients, while utilizing age specific guidelines and standard precaution practices at all times.
- Performs patient teaching for all patients and families.
- Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice to holistically care for the patient.
- In conjunction with team and/or provider, assists in the development and implementation of a comprehensive plan of care.
- Assists providers with on-going assessments and evaluates needs and interventions based on the current status adjusting the care plan as required assuring compliance and understanding of the long-term plan.
- Acts as liaison between the patient, the care team and/or provider.
- Takes thorough and accurate patient history and records pertinent information in medical record as per policy
- Communicates with all staff, patients, and families demonstrating excellent written and verbal skills.
- Administers medications and vaccinations to patients orally, topically, subcutaneously, intradermal and intramuscularly as order by provider or following standing orders policy.
- Assesses information about the patient’s physiological, psychological, sociological, and spiritual status.
- Conducts procedures according to policy while ensuring patient safety.
- Provides health coaching and teaching for common conditions (i.e. asthma, hypertension, diabetes…) through individual nurse visits, group visits and co-visits with other disciplines (clinical pharm, BH, nutrition, physicals…) as appropriate.
- Helps patients stay up to date with clinical care guidelines, immunizations and health maintenance tests
- Triages patients as needed
- Supports transitions of care through nurse visits after ED/Hospitalizations as appropriate
Requirements:
- Successful completion of a nursing program (RN), BSN preferred
- Current MA RN License and current BLS certification required
- Experience in a community health setting, ambulatory care or health insurance environment preferred
- Basic computer literacy, proficiency in Microsoft Office 365 (including Word, Excel, Outlook and Teams), knowledge of EPIC/OCHIN EMR preferred.
- Excellent customer service and verbal and written communications skills
- Ability to effectively work within both a team setting and independently
- Ability to work effectively with economically, racially and linguistically diverse customers
- Bilingual a plus: English and Vietnamese, Spanish, Portuguese or Haitian Creole
