What are the responsibilities and job description for the Intake coordinator position at HARBOR HOMES, INC. DBA HARBOR CARE?
The position is responsible for assisting individuals to gain access to our High Intensity Outpatient Services (HIOP) & Intensive Outpatient Services (IOP), in conjunction with Recovery Housing as well as aftercare planning for when they complete programming.
As a Referral & Intake Coordinator you would provide statewide outreach and education and training on services and eligibility, harm reduction, overdose prevention and other relevant topics to community members, stakeholders and referral sources.
Documenting and tracking data across the 3 areas is also a requirement of this position.
