Wellness Coordinator/LPN/Case Manager

Assisted Living Facility located in Oyster Bay, is currently looking for a Wellness Director/Case Manager/LPN to supervise the care given to our residents. Our ideal Candidate must hold and maintain a current NYS LPN license. Experience with geriatrics in an assisted living facility or long-term care facility preferred.

Candidate must:

· Be able to supervise a staff of resident care associates and med techs.

· Possess excellent leadership and mentoring skills.

· Must have knowledge of DOH regulations and ensure compliance.

· Demonstrates strong ethical and safety practices and must teach and follow all company policies and procedures.

· Must have excellent communication skills and can effectively communicate with staff residents, families, and doctors.

· Must be self-reliant and self-aware and must have the ability to combine common sense with clinical skills to ensure optimal patient care.

· Must Be Vaccinated.

Responsibilities include:

· Able to assess potential residents to determine appropriateness for move-in.

· Able to develop, implement, evaluate, and revise the Plan of Care for each resident.

· Assess continuously the physical and mental needs/conditions of all residents and is always knowledgeable of residents’ conditions.

· Monitors resident medications and ensure all are ordered and received on time.

· Ensure staff follow policies and procedures

· Ensure accurate records are kept relating to medications

· Establishes and Maintains Positive Interactions with residents, families, and Staff.

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
