Harbor’s Edge is a premier 5-star, resort-style Continuing Care Retirement Community (CCRC) located alongside the beautiful harbor in Norfolk, Virginia. Our waterfront community consists of 300 Independent Living Residential Units, a 33-bed (Medicare/private) Skilled Nursing Healthcare Unit, a 33-bed Assisted Living Unit, and 15-bed Memory Support Unit.
We are seeking a positive, motivated, highly skilled full-time LPN for our Clinic to provide excellent nursing services for our Independent Living (IL) residents. Work hours are typically 8:30a-4p, M-F. The new team member selected will have excellent clinical skills and provide appropriate direct nursing care for our elderly residents in a safe, professional, compassionate manner in accordance with scope of practice and HIPAA compliance. Responsible for specified employee health and screening programs in accordance with facility policy and procedures. Ensure compliance with all required federal and state laws, local standards, guidelines, and regulations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- In conjunction with the Health Services Administrator, responsible to plan, direct, and evaluate the activities of the Clinic in compliance with all applicable rules, regulations, policies, and guidelines.
- Provide and document direct nursing care to Independent Living (IL) residents, as indicated following physician orders and established procedures. Arrange for needed equipment and supplies and ensure first aid/urgent care equipment is available and functional. Oversee and perform initial and annual health screenings.
- Oversee maintenance of medical records for IL residents, documenting all pertinent information, and updating information in accordance with facility policy and procedure.
- Track, document and coordinate lab tests, x-rays, and special procedures as ordered by physicians.
- Assist designated physicians in seeing residents during scheduled physician rounds. Make visits as needed and notify designated physician of changes in a resident’s condition, carry out orders received, and monitor effectiveness of treatment.
- Aid with seeking resources from the greater community to ascertain the necessary physician support and other community health resources. Work closely with Social Worker to ensure resident needs are met.
- Communicate with applicable resident’s family or responsible party regarding nursing assessments, treatments, and general care needs, as necessary.
- Provide education, in conjunction with provider group, to residents regarding disease processes, preventive health measures, medication, and diet regimens, as well as facility services.
- Administer employee health and substance abuse screening. Assist in the completion of employee incident reports and workers’ compensation claims and obtain medical care for injured when necessary.
- Manage infection control program for IL residents and employees.
- Administer necessary vaccinations to residents and employees.
- Oversee resident transportation schedule.
- Complete appropriate forms, reports, evaluations, and studies in a timely manner. Participate in surveys and inspections.
- Develop methods for effective coordination of department’s services with other resident services, to ensure continuity of residents’ total regimen of care. Assist with Level of Care changes.
- May assist with other duties as assigned in Health Services areas.
Requirements
REQUIREMENTS:
- High School Diploma or GED equivalent required.
- Current state license as an LPN in the Commonwealth of Virginia or Compact State (state residency compliant).
- Current Adult CPR, BLS/AED certification.
- At least three (3) years of progressive work experience in a Continuing Care Retirement Community (CCRC) long-term care, physician office, home health agency, medical case management role, or related medical environment.
- Serve as a positive role model for service excellence with a strong focus on resident safety and providing excellent nursing care in a timely manner.
- Excellent verbal and written communication, attention to detail, organizational, time management, critical thinking, and multi-tasking skills.
- High self-initiative and ability to accurately deliver and follow verbal and written instructions in a timely manner.
- Skilled at working with the elderly population in a courteous, friendly, professional manner demonstrating patience and compassion. Perform duties with consideration for residents’ rights, demonstrating integrity and discretion in care of residents and their health information.
- Knowledgeable regarding safety protocols, medical and nursing practices and procedures, applicable laws, regulations, and guidelines. Experience with clinical and diagnostic procedures. Able to work independently, make sound nursing judgment, the ability to perform hands-on nursing care, including the delivery of urgent nursing care, as necessary.
- Experience with applicable computer programs and databases including Electronic Medical Record (EMR), Microsoft Office Suite, and ability to effectively operate computers and other office equipment.
- Adhere to Clinic schedule, deadlines and reporting requirements.
