Description

Harbor’s Edge is a premier 5-star, resort-style Continuing Care Retirement Community (CCRC) located alongside the beautiful harbor in Norfolk, Virginia. Our community consists of 300 Independent Living Residential Units, a 33-bed (Medicare/private) Skilled Nursing Healthcare Unit, a 40-bed Assisted Living Unit, and 34-bed Memory Support Unit.


We are seeking a highly qualified, full-time Unit Coordinator to join our team to assist in delivering excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population.


The Unit Coordinator handles clerical and scheduling duties which support Health Services, so as to promote high quality care delivery. This position will consistently demonstrate service excellence and serve as an effective resource for appropriate assistance, knowledge, and information.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Handle telephone calls appropriately, including answering, routing, and taking messages as needed.
  • Responsible for handling various computer data responsibilities including face sheets. 
  • Handle daily resident census update, completing audits as assigned. Ensure all required paperwork is obtained for resident admissions and transfers. Provide support as needed to prepare clinical record documentation for physician and/or nurse practitioner rounds.
  • Manage timely pick and delivery of organized correspondence, communication, and faxed documents. Obtain consult reports and promptly deliver them to nursing leadership in a timely manner. Maintain up to date resident laboratory and diagnostic testing records. Coordinate obtaining test results in a timely manner.
  • Maintain organized files, forms, and departmental documents. Ensure appropriate documents are scanned and filed in accurate resident medical records in a timely manner. 
  • Assist in coordinating resident medical appointments, ensuring completion of required paperwork in a timely manner. Assist to coordinate and/or schedule medical related resident transportation with families.
  • Assist with resident related needs within scope of qualifications and training (e.g., transporting residents for meals and activities, etc.).
  • Perform CNA resident care duties as needed. Provide support to nursing staff during resident mealtimes.
  • Monitor the fire safety alarm panel, follow established protocols, and promptly notify nursing management and Safety and Security personnel as needed.
  • Assist Clinic with maintaining employee health records (e.g., CPR, First Aid, TB, etc.).
  • Responsible for appropriately managing CNA staff scheduling, including maintaining leave requests. Manage schedule in the event of low census staffing needs. As instructed by senior nursing leadership, communicate with external staffing agencies with any short-term staffing needs. 
  • Assist management with documentation of employee callouts and ensure staff members sign attendance sheets per regulatory requirements. Consistently maintain accurate attendance sheets for survey readiness.
  • Crosstrain to assist in maintaining HCC medical supply inventory, including oxygen equipment. Responsible for overall proper inventory management and organization of departmental supplies, equipment, materials, and sanitation. Ensure equipment is consistently in good working order and the department and work areas are organized and neat.
  • Assist with conducting interviews and gathering references for new hires as requested by nursing leadership. Assist with the unit onboarding processes for new staff.



Requirements


  • High School Diploma or GED equivalent. 
  • Current CNA in state of Virginia.
  • Current CPR and First Aid.
  • Minimum of one (1) year of healthcare related work experience. 
  • Prior experience in a long-term care setting preferred, with a strong focus on safety and delivery of high-quality healthcare.
  • Demonstrate strong attention to detail, organizational and time management skills, high self-initiative, and ability to accurately follow and deliver verbal and written instructions in a timely manner. Utilize excellent listening skills.
  • Skilled at working effectively with the elderly population in a courteous and friendly manner, demonstrating patience and compassion. Perform duties with consideration for residents' rights, demonstrate integrity and discretion in care of residents and their health information.
  • Strong service excellence skills, highly organized, adaptable, time management skills, able to multi-task, and effectively meet deadlines. 
  • Consistently demonstrate excellent verbal and written communication skills, communicate in a clear, positive, and engaging manner. Deliver and/or follow verbal and written directives accurately and timely.
  • Knowledgeable regarding medical terminology, medical records management, and various insurance forms and applicable documents related to resident care. Experience with clinical and diagnostic procedures.  
  • Knowledgeable with safe and appropriate use of applicable medical supplies, materials, and equipment, within scope of position.
  • Ability and competence to operate computer, applicable software programs, and other standard office equipment.
  • Perform independently and exercise good judgment. The position is not required to supervise or direct other employees.
  • May be required working beyond normal scheduled hours, on weekends, and in other positions as needed. Expected to report to work on time and work the full established schedule.  
  • Must be able to sit, stand for extended periods of time, stationary positions at certain times during the workday, walk, move about the facilities inside and outside, ascend/descend stairs, reach, turn, bend, stoop, crouch, kneel, balance, maintain body equilibrium to prevent falls, reach overhead, grasp with both hands, fine manipulation, coordination, and repetitive motion. 
  • Must be able to exert at least 50 lbs. of energy in order to assist residents in and out of wheelchairs, beds, etc. Must be able to lift up to 25 pounds unassisted. 
  • Must be able to sit, stand for extended periods of time, stationary positions at certain times during the workday, traverse, move about the facilities inside and outside, ascend and descend stairs, reach, turn, bend, stoop, crouch, kneel, balance, maintain body equilibrium to prevent falls, reach overhead, push and pull, grasp with both hands, fine manipulation, coordination, and repetitive motion.
  • Must have normal eyesight or use corrective glasses/lenses. Must have three-dimensional vision and ability to observe and identify surroundings, judge distances and spatial relationships so as to see objects where and as they actually are, and to recognize colors. 
  • Must be able to distinguish sounds between normal tones, perceive the nature of sounds, comprehend and exchange ideas and detailed information verbally, as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly. 
  • Must have a normal sense of touch and smell. 
  • Ability to operate standard equipment including medical equipment, computers and other office equipment.
