Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, managing treatment plans, and ensuring that patients receive the highest quality of care throughout their healthcare journey. This role requires a strong understanding of various medical conditions, especially in the areas of end of life care, pain management and long term care settings.
Responsibilities
- Assess and evaluate patients’ needs through comprehensive health assessments.
- Develop, implement, and manage individualized care plans tailored to each patient's specific requirements.
- Collaborate with multidisciplinary teams to ensure effective communication and coordination of care.
- Monitor patient decline and adjust care plans as necessary based on clinical outcomes.
- Maintain accurate medical records and documentation in compliance with healthcare regulations. in an electronic medical record suite (Matrix care)
- Educate patients and their families about health conditions, treatment options, and palliative measures.
- Conduct regular follow-ups to ensure adherence to treatment plans and address any emerging health concerns.
- Utilize knowledge of vital signs, physiology, and end of life and pain management practices to provide high-quality nursing care.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in infectious disease care an acute care settings preferred.
- Familiarity with medical records management systems.
- Strong understanding of vital signs monitoring and physiological assessments.
- Excellent communication skills with the ability to work collaboratively within a healthcare team.
- Ability to manage multiple priorities effectively while maintaining attention to detail.
- Previous experience in long term care, home care, highly desired.
- A commitment to providing compassionate patient-centered care.

Pay: $94,775.00 - $98,803.00 per year
