What are the responsibilities and job description for the Transition Coordinator position at Hardeman County Schools?
Transition Coordinator for Transition School to Work Program - Responsible for assisting transition case managers with teaching occupational related skills to eligible and potentially eligible students with disabilities who are interested in employment after leaving high school. The goal is a smooth, seamless transition from high school to post-high school career development and or independent living. Provides job readiness activities in both group and individual settings. Prepares necessary lessons, reports, and information. Coordinates with the Vocational Rehabilitation Counselor, Transition Case Managers and others involved in the IEP development for students with disabilities.
Must have a minimum of a Bachelor degree and experience working with Students with disabilities.
