Opportunity Available
This role is a nursing case manager (NCM) who serves vulnerable children and families in Marion and surrounding counties. The NCM provides education and skills training to families and children to ensure compliance with healthcare needs. As an independent contractor, you will set and control your own schedule.
To be considered for this role, you must be a nurse holding an unencumbered license in Indiana, have at least one year of long-term care experience, have an updated physical and 2-step PPD, be CPR certified, hold a valid driver's license and auto insurance, and be able to pass our comprehensive background screening process.
Who We Are
We are an accredited community college that provides educational courses and services geared towards healthcare. We work directly with community organizations and local school systems to provide education and training in certificate programs to students of all ages. Further, we work with local school systems to provide school nursing services. Job Responsibilities
- Develop detailed nursing plan of care based on in-depth assessment of living environment
- Educate family and youth on proper management of diseases and medical conditions
- Coordinate with members of the multi-disciplinary team to meet client's holistic needs
- Oversee complex medical needs of clients
