What are the responsibilities and job description for the Medical Case Manager position at Harm Reduction Services?
Introduction: The Ryan White HIV/AIDS Program is a federal program that provides HIV care, treatment, and support services to people with HIV. It's the largest federal program in the United States for people with HIV. The Harm Reduction Services Ryan White Medical Case Manager will work up to 40 hours per week as assigned on the Ryan White Case Management program.
The job involves travel throughout the Sacramento region, including street outreach work. The hours will be determined weekly to cover the needs of clients on the Ryan White case load. Tasks may include attending various training's, including the State Office of AIDS’ HIV test counselor certificate program (if not already certified), participating in health-related outreach and resource development, navigating the systems of care both medical and social specifically benefits support and food access. This position requires employee to be in client's personal environments such as homes, medical offices’, homeless camps, as well as transporting clients in employee personal vehicle. This job requires use of employee’s own vehicle daily and therefore requires that employee has a reliable vehicle, with appropriate automobile insurance, a valid California driver license, and a safe driving record. The worker may be assigned to operate vehicles owned by the agency as well.
The job requires travel and direct work with clients who may use drugs, have mental illness or other chronic diseases and/or difficulties, and interaction with public and private organizations. The job requires a background search for criminal arrests and convictions. A criminal record does not necessarily disqualify an applicant, each case will be considered on its own merits. The job responsibilities include lifting up to 40 pounds, cleaning, walking up to a mile several times a day, climbing stairs and being comfortable in street settings throughout the community.
Duties will include risk assessment, client counseling, HIV/HCV/STD testing, service referrals, client interviews, data collection, confidential record keeping, client recruitment, management of resources, interaction with many people and the coordination of this program’s activities with other services from HRS and others in the community.
Basic Requirements:
- Physical health that allows the employee to use public and private transportation, to walk distances up to 1 mile several times a day and work their assigned tasks for periods up to 8 hours per day with appropriate breaks. Duties may occasionally include lifting up to 40 pounds, climbing a flight of stairs and light cleaning of the office area.
- Strong writing skills that allows the worker to complete record keeping forms necessary to the data collection and reporting associated with this project. This will involve report writing and some grant requested tasks. Experience with data collection, entry and analysis that will allow the individual to complete and maintain data collection instruments and systems. Ability to develop relationships with clients and other providers is key to the program’s success.
- Effective communication and problem-solving skills and experience dealing with diverse individuals and communities. Possess the skills necessary to display client-centered, non-judgmental services to all prospective clients, and to participate in planning and coordination meetings with others from the service community.
- Reliable, personal vehicle with appropriate vehicle insurance, a valid California driver license, and safe driving record. Employee will be reimbursed for mileage at the national IRS rate (currently 70 cents/mile).

Required Tasks
The worker will perform all of the following tasks:
- Report to work on time and complete all assigned hours and tasks. Must work a full 8-hour shift between the hours of 7am-6pm based on client needs. Assigned hours may include weekend and evenings. If the worker is not able to report to work for any reason, they are to contact their supervisor by phone in advance to say that they are not available for work. Poor attendance or the lack of achievement of assigned objectives will be cause for employee counseling sessions and other personnel action, including termination, as necessary.
- Coordinate the scheduling with case management team of the weekly events and related activities contributing to program success with the staff team.
- Be responsible for assigned record keeping and data reporting, including design, modification and the operation of data systems necessary for their project responsibilities. They are also responsible for the management and data collections involved with accounting for achievement toward assigned program goals.
- Work with all clients following the agency’s guidelines, which require non-judgmental, caring interactions. All HRS staff, interns and volunteers who are associated with the agency will refrain from inappropriate interactions with clients, including buying or selling items, lending money or participating in personal relationships outside of the workplace or business hours.
- Conduct client data collection and on-going contacts in support of the program’s effective operation. This will include following up on referrals made to other service providers and re-contacting clients to follow up on services given as necessary.
- Safe collection and handling of body fluids collected for disease testing.
- Collect and enter data to be used in the evaluation of program effectiveness and work with others in creating evaluation reports for use in assessing project accomplishments.
- Collaborate with all staff of other community agencies in providing connected and effective services that support other’s efforts in providing quality care.
- Provide training and direction to peer volunteers who work with this program.
- Attend all assigned staff and team meetings and participate appropriately in the planning and review of project activities.
- Maintain client files in an acceptable manner, with regular updates and required reports to Supervisor.
- Prepare and submit monthly billing reports of activities conducted with clients and submit them to Supervisor on time.
- Complete other appropriate tasks as assigned by supervisors in support of all HRS activities.
Employment conditions:
- This employee reports directly to the Ryan White Director and will receive their performance evaluations and scheduling from that person. This person will submit time sheets, mileage reimbursement forms and program reports to the Ryan White Director.
- Harm Reduction Services is an at-will employer. Personnel management and discipline, including termination, will be done in accordance with all applicable laws.
- The Ryan White Director will make all decisions regarding hiring, promotion, increase or reduction in assigned hours and termination. Decisions regarding employment, promotion, raises, termination, and all other employee benefits will be based on programmatic considerations and the required activities of the agency and its specific needs. Considerations of seniority, length of service for the agency or in discharging specific duties may be considered in the decision process but will not govern the outcome of these decisions.
- Employees must annually provide copies of their valid California Driver’s License and insurance for personal vehicles to be used in work related driving.
- Compensation:
Expenses for the use of a personal vehicle for assigned travel will be reimbursed at $.70/mile. HRS assumes no other liability for the use of automobiles in performing job tasks.
The hourly wage for this position is $26.00/hour. We offer 18 Holidays/ year, PTO, sick time, fully paid health, dental and vision benefits, matching retirement plan, scheduled pay raises.
