The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. JOB TITLE: Psychiatric Nurse Practitioner- Per Diem
REPORTS TO: Medical Director
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:
- Evaluate, diagnose, and treat persons with mental disorders.
- Completes the history of the present illness and review past histories and social work evaluations
- Interview patients to obtain insight into their problems.
- Make referrals for further diagnostic psychological or medical testing as needed.
- Prescribes psychiatric medication when needed
- Observes and differentiates between signs and symptoms associated with normal human physiology and those indicative of pathological changes.
- Screen patients for physical illness and analyze explanations for psychiatric illness, such as developmental issues, family dysfunction, genetics, neurological trauma or social or cultural problems.
- Involves with direct patient physical assessment, education, and health planning.
- Participate in LEAN and other activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of pediatric patients
- Maintains continuity of care by regular patient follow-up.
- Assists in the coordination of services with other disciplines and patient’s insurance plans.
- Advocates for patient in arrangement of medically necessary services.
- Communicates with community’s resources to ascertain adequate assistance for patients in need.
- Refers patients to specialty services when necessary.
- Consults with collaborating physician and other members of the health care team to establish plan of care.
- Participates in quality assurance process to assure optimal care delivery.
- Provides comprehensive case management services that include emphasis on health promotion and disease prevention.
- Monitors patient outcome in accordance to the practice protocols;
- Collaborates with staff nurses and nursing administrators with assessment and clinical decision in patient care planning.
- Acts as a resource person for the nursing staff.
- Conducts education in service on practice issues.
- Other duties as assigned.
QUALIFICATIONS:
- Graduated from approved Nurse Practitioner Program.
- Licensed as Nurse Practitioner in New York State.
- Basic CPR (BCLS) certification required.
- Specialty Certification Preferred.
- Bilingual in English and Spanish or Creole preferred.
SALARY: Commensurate with experience
