The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. JOB TITLE: Licensed Clinical Social Worker (School Based Health Center)
REPORTS TO: Director of Behavioral Health
This position is located primarily within the schools in which Harmony Healthcare Long Island has School Based Health Centers. The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:
- Conducts psychosocial assessments and develops and implements treatment plans
- Provides health services in accordance with Principles and Guidelines for School Based Health Centers in NYS
- Responds to referrals and consultation requests from primary care provider(s)
- Prepares psychosocial histories/reports and maintains treatment records, in accordance with clinic and regulatory guidelines
- Review student records to identify previous barriers and/or interventions
- Provide referrals to health care resources and help locate financial and legal assistance in the community.
- Provide individual and/or group counseling in response to school-wide crises
- Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services.
- Participate in professional development activities aimed at current trends and best practices for the provision of comprehensive school social work services
- Maintain an ongoing liaison with community agencies and other resources to meet student needs
- Participates in agency Quality Assurance activities and case discussion meetings, as appropriate
- Other duties as assigned.
QUALIFICATIONS:
- Master’s Degree in Social Work from an accredited school of social work
- New York State License required and Clinical designation
- Strong verbal and written communication skills
- Previous work experience in adolescent behavioral health required
- Ability to travel to HC sites if needed
- Bilingual in English and Spanish/Creole preferred.
SALARY: Commensurate with experience
