The Harmony Healthcare Long Island, formerly known as (Long Island Federally Qualified Health Center or LIFQHC) is a non-profit healthcare organization with 7 health centers, providing primary care and preventative medicine in the following locations in Nassau County: Roosevelt, Elmont, Hempstead, Freeport, Oceanside, and New Cassel/Westbury. In addition, the Harmony Healthcare Long Island has 3 school-based health centers, WIC offices (Special Supplemental Nutrition Program for Women, Infants, and Children) in 3 locations, and a Health Home Care Coordination program. JOB TITLE: Licensed Clinical Social Worker
REPORTS TO: Director of Behavioral Health
The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.
RESPONSIBILITIES:
• Conducts psychosocial assessments and develops and implements treatment plans
• Obtains collateral information from patients, family members, outside treatment providers, schools, and other relevant team in the assessment of medical and behavioral health needs
• Provides therapeutic intervention to patients with behavioral health needs, including individual, family, and group psychotherapy, community work, crisis intervention, case management, other treatment modalities
• Responds to referrals and consultation requests from primary care providers
• Prepares psychosocial histories/reports and maintains treatment records, in accordance with clinic and regulatory guidelines
• Participates in agency Quality Assurance activities and case discussion meetings
• Provides task supervision and consultation to social work interns, other clinical trainees, and volunteers if needed
• Other duties as assigned.
QUALIFICATIONS:
• Master’s Degree in Social Work from an accredited school of social work
• New York State License required
• Strong verbal and written communication skills
• Previous work experience in behavioral health or health setting preferred
• Ability to travel to other sites as needed
• Bilingual in English and Spanish required
SALARY: Commensurate with experience
