Position Overview:
We are seeking a compassionate, dedicated, and skilled Shift Care LVN to join our healthcare team. As an LVN, you will provide nursing care to patients in a variety of settings under the supervision of registered nurses and physicians. This position requires flexibility with shifts, and the ability to manage a range of patient needs and duties in a dynamic, fast-paced environment.
Key Responsibilities:
- Direct Patient Care: Provide nursing care to patients by administering medications, dressing wounds, monitoring vitals, and assisting with daily living activities.
- Patient Assessment: Regularly assess patient conditions, report significant changes to the supervising RN or physician, and adjust care plans accordingly.
- Collaboration: Work closely with other healthcare providers to ensure continuity of care and comprehensive patient treatment plans.
- Documentation: Maintain accurate and up-to-date patient records, including health histories, care plans, treatments, and progress notes.
- Family Interaction: Provide guidance and support to patients’ families as needed, educating them on patient care procedures and health management.
- Shift Handover: Ensure smooth transition between shifts by thoroughly communicating patient updates, needs, and progress during shift changes.
- Safety Compliance: Ensure compliance with infection control protocols, patient safety regulations, and facility policies.
- Emergency Response: Respond promptly to medical emergencies and escalate to higher-level staff as necessary.
Qualifications:
- Licensure: Current, valid LVN license in the state of [Insert State].
- Education: Graduate of an accredited nursing program.
- Experience: Previous experience in a healthcare setting, preferably in a shift-based or long-term care environment.
- Skills:
- Strong clinical assessment and patient care skills.
