Medical Hospice Social Worker

Job Overview:

We are seeking a compassionate and dedicated MSW Social Worker to join our team. The ideal candidate will provide essential support and advocacy for patients in need, focusing on medical documentation, behavioral therapy, and case management in a clinic setting.

Responsibilities:

- Conduct patient assessments to identify needs and develop individualized care plans
- Provide behavioral therapy and counseling to individuals and groups
- Collect and maintain accurate data for patient records and program evaluation
- Collaborate with healthcare professionals to coordinate diagnostic evaluations and treatment plans
- Manage cases involving pediatric patients, dementia care, and other specialized populations
- Offer support through crisis intervention and emotional assistance

Experience:

- Must have a Master's degree in Social Work (MSW) or be a Licensed Clinical Social Worked (LCSW)
- Proven experience in medical documentation, patient assessment, and case management
- Familiarity with individual and group counseling techniques
- Knowledge of diagnostic evaluation processes and treatment planning
- Experience working with diverse populations including pediatrics and individuals with dementia
- Strong communication skills and ability to collaborate effectively with interdisciplinary teams
