Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home care team. The LPN will be responsible for conducting patient intakes and supervising care staff as needed. This role is essential in ensuring high-quality care for our patients in their homes while supporting and guiding caregivers to deliver exceptional services.
Key Responsibilities:
Patient Intakes & Assessment:
- Conduct initial and follow-up assessments to evaluate patients' medical conditions, needs, and care plans.
- Collect patient history, vital signs, and current medication lists.
- Work closely with the Registered Nurse (RN) to develop individualized care plans.
- Educate patients and families on the care plan, medications, and home care services available.
2. Supervision of Care Staff:
- Oversee Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), and other care staff to ensure compliance with patient care plans and regulations.
- Ensure that caregivers follow infection control, safety, and documentation protocols.
- Assist in scheduling and coordinating patient visits with care staff.
3. Providing Nursing Services:
- Medication Management
- Provide education and support to patients and families regarding disease management, medication adherence, and overall wellness.
4. Documentation & Compliance:
- Maintain accurate and up-to-date patient records using the electronic medical records (EMR) system.
- Document all nursing interventions, changes in patient condition, and supervisory visits.
- Participate in quality improvement initiatives, audits, and team meetings to enhance patient care.
