POSITION SUMMARY: Responsible for providing personal care services, assistance, support, and supervision to patients.
REPORTS TO: RN/LPN
REPONSIBILITIES:
- Adheres to and conveys a philosophy that supports the dignity, privacy, independence, choice, and individuality of patients.
- Follows patient care plans in providing services to patients.
- Assist patients with activities of daily living
- Assist patients with personal care, incontinence care, mobility deficits
- Provide companionship, emotional and spiritual support to assigned patients
- Perform light housekeeping duties per plan of care
- Serves meals per plan of care
- Assist patients with oxygen, catheters, and other medical needs per care plan
- Monitor the health, nutritional, hydration, safety and overall well-being of patients report concerns to supervisor
- Document information pertaining to changes in patient’s functional status, personal needs, appointments, etc.
- Maintain documentation of each visit and tasks performed as per policy
- Assist in keeping the environment clean and safe for patients.
- Report concerns about patients to supervisor and co-workers.
- Respond to patient emergencies and arrange for appropriate medical attention.
- Identify supplies needed to perform tasks and report needs to supervisor.
- Practice safety in working and with equipment, chemicals, tools and utensils.
- Adhere to and carry out all policies and procedures.
- Establish and maintain constructive work relationships with co-workers, patients, and families.
- Maintain confidentiality of verbal and written information pertaining to patient, facility operations, and personnel.
- Promote Unity Hospice in a positive manner.
- Performs all other duties as assigned.
EDUCATION AND QUALIFICATIONS:
Must have a demonstrated ability to effectively communicate verbally, with the ability to read, write and understand English. Is able to regularly lift up to 30 pounds, to occasionally lift more than 30 pounds, and to spend long periods of time walking/standing. Preference will be given to candidates with 1-2 years prior experience working with the elderly, preferably in a patient setting. Must have training and experience as a nursing assistant and hold a New Hampshire state licensure as a LNA. Must be at least 18 years of age, with a high school degree or equivalent GED preferred.
Salary : $18 - $25
