What are the responsibilities and job description for the Street Outreach/Coordinated Entry Specialist position at Harmony House?
General Description of Duties and Responsibilities
- Identify and build rapport with persons experiencing unsheltered homelessness in the Cabell and Wayne County area.
- Assess the individual needs of those experiencing homelessness in the Cabell and Wayne County area.
- Effectively share information about services available through the Continuum of Care and make referrals to appropriate programs.
- Coordinate ongoing outreach efforts and respond to requests for assistance from the community.
- Provide individualized support, using evidence-based case management tools to assist with developing a plan to address barriers that prevent persons experiencing homelessness from accessing permanent housing.
- Coordinate, cooperate and communicate with agencies serving participants in the Continuum of Care
- Provide crisis intervention services as needed
- Operate as a member of the Harmony House team, striving to always provide quality services to persons experiencing homelessness, including those experiencing serious mental illness and/or substance use disorders.
- Utilize 3 evenings a week (4p-9p) conducting Coordinated Entry Assessments for unsheltered clients or on as needed basis.
- Attend weekly staff meetings.
- Other responsibilities as assigned by supervisors.
Must be available to work some evening and weekend hours.

Qualifications for Position
High school diploma or GED; Lived experience preferred, but not required.
Valid driver’s license required.
Pre-employment drug screen.
Supervisory Relationships: There are no supervisory responsibilities for this position.

Amount of Travel: Local travel required
