What are the responsibilities and job description for the Case Manager- Substance Use Disorder position at Harmony Recovery?
Primary Function: In collaboration with clients, effectively develop plans to link appropriate services in addressing individual specific needs for an effective transition post discharge.
Supervision: Case Management Team Leader
Essential Functions:
- Meet with clients throughout their stay to collaboratively determine continuing care needs in a structured and simplified manner.
- As appropriate, consult with family members in identifying collateral information critical to removing potential gaps in treatment.
- Work to integrate support for clients in a comprehensive manner to address medical, social, and mental health needs.
- Establish and maintain effective working relationships with referents and family members through regular follow-up.
- Participate as an active member of the treatment team coordinating post discharge plans with Counseling and Medical teams.
- Facilitate communication between clients and continuing care resources to ensure smooth transition to the next level of care.
- Participate in conflict resolution and crisis interventions with clients as needed.
· Facilitate transfer and discharge processes in a timely manner and in coordination with Nursing and Counseling Technicians.
- Complete all necessary documentation and record keeping with accuracy and attention to detail.
· Adhere to confidentiality laws including 42CFR Part 2, HIPAA and HITECH as well as all Harmony policies and procedures.
Minimum Requirements:
· Bachelor’s degree in Psychology, Social Work, or equivalent work experience is preferred.
· Certified Addiction Technician designation or NADAAC equivalent is preferred.
· Must have valid Colorado driver’s license and acceptable motor vehicle record if driving Harmony vehicle or driving for Harmony business.
· Computer skills adequate for required tasks. Must be proficient in Microsoft Office.
· Previous work experience in mental health or substance use disorders.
Knowledge, Skills and Abilities:
· Ability to relate with detachment, understanding, and empathy to individuals with substance use disorders and their families.
· Ability to communicate effectively in written and verbal formats.
· Excellent motivational interviewing skills and an ability to establish a therapeutic alliance with clients.
· Skilled in identifying psychosocial stressors for clients to anticipate appropriate services.
· CPR/First Aid certification.
· Demonstrated knowledge and understanding of addiction and mental health issues.
· In-depth knowledge of pragmatic and immediately useful resources for clients.
· Skilled in presenting information in a culturally competent and non-stigmatizing manner.
· Personal maturity as reflected by sound judgment and decision-making.
· Ability to work as a positive and collaborative team member.
