Position Overview:
The Case Manager at Harmony Women's Recovery plays a vital role in supporting women navigating early sobriety. This position focuses on providing individualized assistance with health, legal, and stability-related challenges, empowering clients to rebuild their lives and achieve sustained recovery.

Key Responsibilities:

  • Client Support & Advocacy:
  • Develop and maintain supportive, professional relationships with clients.
  • Serve as an advocate for clients in accessing health, legal, and social services.
  • Collaborate with clients to create individualized plans for addressing stability-related needs, such as housing, employment, and education.
  • Health and Wellness Coordination:
  • Assist clients in connecting with medical and mental health care providers.
  • Monitor client adherence to treatment plans and facilitate communication between healthcare providers as needed.
  • Educate clients on the importance of maintaining physical and mental wellness in recovery.
  • Legal Assistance:
  • Guide clients in navigating legal challenges, such as court-mandated obligations or family law issues.
  • Provide referrals to legal resources, including attorneys or legal aid organizations.
  • Support clients in maintaining compliance with probation or parole requirements.
  • Resource Management & Stability Planning:
  • Connect clients with resources for housing, food, transportation, and other essential needs.
  • Assist with job placement, vocational training, and educational opportunities.
  • Provide support with financial planning, including budgeting and managing expenses.
  • Documentation and Reporting:
  • Maintain accurate and timely records of client interactions, progress, and outcomes.
  • Prepare reports for internal use and external compliance as required.
  • Ensure all documentation adheres to confidentiality and professional standards.
  • Collaboration and Teamwork:
  • Work closely with clinical and housing teams to ensure holistic support for clients.
  • Attend team meetings to discuss client progress and share insights.
  • Provide feedback to improve program services and client experiences.

Qualifications:

  • Certified Chemical Dependency Counselor Assistant (CDCA) or greater.
  • Minimum of 2 years of experience in case management, substance use treatment, or a related role.
  • Strong knowledge of addiction recovery principles, resources, and best practices.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to build rapport with diverse populations and maintain professional boundaries.

Preferred Qualifications:

  • Familiarity with the 12-step recovery model.
  • Experience working in substance use treatment or women-focused programs.
  • Knowledge of local resources for health, legal, and social services.

Work Environment:

  • Flexible schedule may include evening or weekend availability.
  • Primarily office-based with some community outreach and home visits.

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: 40 per week
