Responsibilities
The Licensed Professional Nurse/Licensed Practical Nurse provides intermittent skilled nursing services to patients in the home setting according to the patient’s plan of care. As a Licensed Professional Nurse/Licensed Practical Nurse you will:
- Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities, including activities of daily living as outlined in the patient Plan of Care
- Submit completed skilled nursing visit notes as requested by Clinical Supervisor
- Participate in discharge planning process per Medicare Criteria
- Attend staff meetings, team conferences and educational in-services as required
- Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications
- A current unencumbered State Professional Nurse License
- Active CPR Certification
- Must maintain a valid driver's license, reliable transportation, and good driving record
- The ability to make sound professional clinical judgment
- The ability to assess and document patient needs and meet those needs
- Proficiency in Microsoft Office
