\Job Responsibilities
\As a Hospice Medical Social Worker, you will play a vital role in our interdisciplinary team. Your responsibilities will include:

- Providing social casework to patients and their families, addressing their unique needs and concerns \
- Collaborating with physicians, nurses, and other healthcare professionals to develop comprehensive care plans \
- Identifying and referring patients to community resources and support services \
- Participating in team conferences to discuss patient progress and develop strategies for improving care \
- Maintaining accurate records and documenting patient interactions and outcomes \
- Staying current with industry developments and best practices in palliative care and social work \
- Contributing to a positive and supportive work environment that values teamwork and mutual respect \
- A Master's degree (MSW) from a school of Social Work accredited by the Council on Social Work Education \
- Two years' experience working in hospice care, preferably in a similar role \
- Advanced Palliative Hospice Social Worker Certification-APHSW-C, through Social Work and Palliative Care Network \
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations \
- Proficiency in using electronic health records and other relevant software applications \
