Job Responsibilities
The Registered Nurse will assess and evaluate patient needs, identifying mutually agreed upon goals for care planning. They will report patient status, updates, and discharge planning to clinical leadership, attending physicians, and hospice physicians. The RN will also attend staff meetings and educational in-services as needed, participate in Quality Assessment and Performance Improvement (QAPI) program, and ensure the use of the 4Ms and provide Age-Friendly Care.
Key Qualifications
- Current unencumbered registered nurse licensure in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
- Maintain a valid driver's license and good driving record
- Ability to work in a field setting and make sound nursing judgments
- Assess patient needs and formulate individualized patient care plans
- Experience with Microsoft Office
