Job Responsibilities
The Nurse Practitioner In-Home Assessment is responsible for performing health risk assessments and transitional care for patients discharged from hospitals to prevent re-admissions. Key responsibilities include:
- Conducting comprehensive in-home health risk assessments to identify all active and chronic disease conditions
- Providing education and counseling to patients and families based on assessment results
- Performing follow-up visits in transitional care to ensure optimal patient outcomes
Qualifications
To be successful in this role, you must possess:
- Current RN and NP licensure in the state of practice
- Current CPR certification
- Enrollment in both Medicare and Medicaid
