About the Role
The Speech Language Pathologist will work closely with our interdisciplinary team to provide comprehensive care to patients in their homes. The successful candidate will be responsible for assessing and evaluating patient needs/problems, identifying mutually agreed-upon goals with patients for patient care plans, and providing speech therapy services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient care plan. Additionally, the Speech Language Pathologist will report patient status to Clinical Supervisor and referring Physician, document daily changes and updates to the patient care plans, including discharge planning, attend staff meetings, team conferences and educational in-services per agency requirements, and perform all OASIS time point assessment per Medicare Criteria and submits recertification paperwork per policy and procedure.
