Position Overview:
Under moderate supervision, the ACCESS Healthy Start Liaison Specialist will focus on shared populations needing services and support across the safety net continuum, including but not limited to the following cohorts: violence prevention, black maternal health, homeless individuals with physical and behavioral health conditions, re-entry populations, and the Latino chronic disease cohort. Individuals in this role will facilitate coordination between ACCESS cohorts, build collaborative relationships with stakeholders, and monitor program performance to ensure effective care coordination and outcomes.

The ACCESS Healthy Start Specialist is expected to have strong interpersonal skills, a thorough understanding of community health challenges, and the ability to work collaboratively with diverse populations.

Duties & Responsibilities:

  • Care Coordination: Participates in developing care coordination strategies in partnership with ACCESS Cohort Managers. This includes providing case management services for participants who have not enrolled in an ACCESS Harris Cohort, aiding participants with internal and external referrals for medical, substance use, mental health, parenting, employment, housing needs, and other needs related to social determinants of health, and working with participants to identify and address safety concerns and create a plan for long-term safety and well-being.

  • Collaboration: Fosters and manages collaborative relationships with county safety net departments and community-based organizations.
  • In-field Support: Attends medical, housing, and other needed appointments and any additional in-field support requested by participants. Communicates effectively and efficiently with community members, colleagues, and clients.
  • Assessments: Conducts site and home visits as needed to complete initial assessments on new participants and provide follow-ups to existing participants. This includes documenting detailed client case notes and overall coordination of all staff reports and ensures accurate reporting and maintenance of program data and service deliverables.
  • Trauma Case Management: Provides trauma-responsive case management services including empathetic advice and guidance to participants to ensure participant satisfaction and program completion, acting as an advocate to help participants make informed decisions regarding their clinical, emotional, nutrition, and life skills status, and assist them in securing access to and independently mastering community services and resources.
  • Performance Support: Assists and ensures that continuous quality improvement strategies and management performance to achieve outcomes of crucial program components.
  • Reporting: Prepares a variety of reports and correspondences ensuring data collection occurs and program evaluation of outcomes is completed. In addition, engages with the community and stakeholders informing and facilitating partnerships related to key initiatives, as well as providing presentations to various audiences as needed.

Education:
  • Bachelor's degree in business administration, Public Administration, Public Health Administration, or related field.
Experience:
  • At least one (1) year of experience working as Case Manager or in Case Management.
  • At least one (1) year of experience working with coordinate care.
Licenses/Certifications Required:
  • Valid Driver's License (Texas upon hire) along with a good driving record required.
Knowledge, Skills and Abilities:
  • Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • Experience documenting case notes.
  • Knowledgeable of community resources and refer clients as needed.
  • Demonstrates an awareness of, and sensitivity to, the needs and concerns of individuals from different cultures.
  • Ability to communicate effectively and appropriately, both written and oral.
  • Ability to work independently and meet reasonable deadlines.
  • Ability to handle sensitive or difficult issues with grace and confidence.

Education:
  • Master's degree in business administration, Public Administration, Public Service & Administration, Health Administration, or related field.
Experience:
  • At least one (1) year of supervisory/management experience.
  • At least one (1) year of case management software experience.
Language:
  • Bilingual (English/Spanish). 

Position Type and Typical Hours of Work:
  • This is a full-time position; 40 hours per week (including evenings and weekends).
    Ability to work outside regular business hours (evenings and weekends) is required.
    Requires travel which includes transporting clients, participating in community outreach programs, and attending meetings and training opportunities.
Work Environment:
  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. Will conduct visits to participants homes using the agency vehicles and attends periodic events in the community.
Physical Demands:
  • Frequently required to sit or stand, use manual dexterity, speak, listen, hear, and write. Works in an office or indoor environment with little or occasional light physical effort and moderate noise. May occasionally lift and move up to 50 pounds.
