This position is responsible for providing social services support as requested by the physicians and other Medical staff and to perform counseling and demonstrate the ability to provide positive and supportive communications to patients and families. Key responsibility is to facilitate the smooth transition of patients from one level of care to another using available resources. Participation in the multi-disciplinary team meetings is another responsibility of this position.
