Hart to Hart Home Care is seeking a compassionate and dedicated Personal Care Assistant to provide essential support to individuals in need of assistance with daily living activities. Your role will be to assist clients with daily living activities, ensuring their well-being and providing companionship in a warm, supportive environment. The ideal candidate will have experience in memory care, damentia care, enjoy working with seniors and their families, and share our vision of serving with care, integrity, and heart!
Our clients reside in the following communities: Clayton, Dekalb, Henry and Spalding counties.
Responsibilities:
- Assist clients with personal hygiene tasks, including bathing, grooming, and dressing.
- Provide companionship and engage clients in meaningful activities to enhance their quality of life.
- Monitor and document changes in clients' physical and emotional well-being through patient observation.
- Ensure compliance with resident rights and maintain confidentiality at all times.
- Other medically related activities when assigned.
- Proper nutrition.
- Home management - (Safety Prevention).
- Ambulation and transfer.
- Light Household tasks.
- Medication reminders.
- Accompanied medical visits (PRN).
Requirements:
- PCA must have 3 years of PCA experience.
- Certified Nursing Assistant (CNA) License: Preferred.
- CPR and First Aid Certification: Required for employment.
- Clear Background Check: Must pass a criminal background check, drug screening, and a driving record check.
- TB Screening: Must have negative results.
- Knowledge of memory care techniques and dementia care practices.
- NLN Exam on line and assessment must be completed for services performed or completion of an approved 40-hour training curriculum and assessment of competency.
- Annual evaluations: The supervisor conducts annual evaluations to assess the competencies of the caregiver through direct observation or demonstration, on the job tasks the caregiver is required to perform. The caregiver will sign and date their evaluation and it is maintained in each file.
- Valid driver’s license, auto insurance and reliable transportation.
- Ability to lift and/or move up to 50 pounds with physical capability to perform job-related duties.
- Must be able to follow verbal and written instructions, complete written reports, demonstrate understanding and practical competency in understanding the needs and characteristics of the elderly, handicapped, or convalescing individuals.
- Hoyer Lift Experience (specific to client).
