Job Shift: Shift 1
Shift Details: Monday -Friday 8-4:30 , one weekend a month., 1 major and 2 minor holidays per year. Coverage area is for Valley pods for Watertown office

Job Description
- Facilitate communication between the patient and the interdisciplinary team.
- Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
- Utilize state-of-the-art technology to document skilled services provided.
Qualifications
- Licensed Practical Nurse with Valid CT LPN license
- One year of LPN experience; Homecare experience preferred.
- Valid driver's license and reliable transportation required.
- Computer proficiency required.
