About the Role:
We are seeking a highly skilled Nurse Assessment Specialist to join our team as an MDS Care Coordinator. In this role, you will provide oversight of the Resident Assessment Instrument (RAI) process and conduct assessments and care plan coordination for our residents.
Responsibilities:
- Ensure accurate and timely completion of MDS, CAA's, and care plans
- Coordinate scheduling of the RAI process with the interdisciplinary team
- Conduct thorough assessments of residents, including physical assessment, interview, and chart review
- Collaborate with caregivers to discuss resident care needs and develop effective care plans
- Stay up-to-date with AANAC requirements and maintain knowledge of the RAI process and PPS
Qualifications:
- Registered Nurse (RN)
- AANC certification a plus; RAC-CT
- Knowledge of the RAI process and PPS
