Job Overview:
Haven Home Health seeking a dedicated and skilled Admission RN to join our home health team in the Tampa area. This full-time role focuses on conducting 2-3 Start of Care (SOC) visits per day, ensuring high-quality patient-centered care. The ideal candidate will have strong clinical assessment skills and experience with OASIS documentation.
Key Responsibilities:
- Perform Start of Care (SOC) assessments for new home health patients.
- Develop and implement individualized patient care plans in compliance with regulations.
- Utilize OASIS documentation and Kinnser EMR for accurate and timely charting.
- Educate patients and families on care plans, medication management, and disease processes.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Ensure compliance with Medicare guidelines and home health policies.
Qualifications:
- Registered Nurse (RN) license in the state of Florida (required).
- 1 year of home health experience preferred (willing to train the right candidate).
- Proficiency in OASIS and Kinnser EMR (preferred but not required).
- Strong organizational and time-management skills.
- Valid driver’s license and reliable transportation (mileage reimbursement provided).
