Job Description:
We are seeking a dedicated and skilled Hospice Admission Registered Nurse (RN) to join our home hospice team in Tucson, AZ. As an Admission RN, you will play a crucial role in onboarding new patients, conducting comprehensive assessments, and initiating personalized care plans to ensure a smooth transition into hospice care. This position is ideal for a compassionate nurse who excels in patient education, clinical assessments, and providing support during the admission process.
Responsibilities:
- Conduct in-home admissions for new hospice patients, including initial assessments, and explain hospice services and care plans.
- Collaborate with physicians and the interdisciplinary team to develop and initiate individualized care plans.
- Provide education to patients and families on hospice philosophy, services, and symptom management.
- Ensure accurate and thorough documentation of patient assessments, care plans, and other pertinent information in electronic medical records.
- Serve as a primary point of contact for patients and families during the admission process, addressing questions and concerns with empathy and clarity.
- Coordinate with other hospice team members to ensure continuity and quality of care.
- Maintain compliance with all regulatory and hospice standards during the admission process.
Qualifications:
- Current RN license in the state of Arizona.
- Minimum of 1-2 years of hospice, palliative care, or relevant clinical experience preferred.
- Strong assessment, clinical, and critical thinking skills.
- Excellent communication and interpersonal skills, with the ability to effectively educate and support patients and families.
- Compassionate, patient-centered approach with a commitment to quality care.
- Valid driver’s license, reliable transportation, and current auto insurance.
