Job Summary:The Hospice Chaplain provides spiritual support, counseling, and guidance to patients, families, and staff within the hospice care setting. Key Responsibilities:
- Spiritual Care and Counseling:
- Provide spiritual support and counseling to hospice patients and their families.
- Offer spiritual assessments and develop individualized care plans in collaboration with the interdisciplinary team.
- Conduct regular visits to patients and families, offering emotional and spiritual support.
- End-of-Life Support:
- Assist patients and families in coping with terminal illness, grief, and bereavement.
- Conduct funerals, memorial services, and other end-of-life rituals as requested.
- Provide crisis intervention and support during critical moments.
- Interdisciplinary Team Collaboration:
- Participate in interdisciplinary team meetings to discuss patient care plans and provide insights on spiritual needs.
- Collaborate with medical professionals, social workers, and other team members to ensure holistic patient care.
- Document spiritual care services provided in patient records.
- Education and Training:
- Educate patients, families, and staff about the role of spiritual care in hospice.
- Offer training and support to staff on issues related to spirituality, grief, and end-of-life care.
- Community and Volunteer Engagement:
- Build relationships with local faith communities and clergy to enhance the support network for patients.
- Recruit, train, and supervise volunteer spiritual care providers when necessary.
Qualifications:

- Experience: Minimum of 2 years of experience in pastoral care, preferably in a healthcare or hospice setting.
