This position plays a vital role in supporting our clinical team and managing patients within an Accountable Care Organization (ACO) framework.

Responsibilities:
- Manage and coordinate care for patients within the ACO to achieve optimal clinical outcomes and cost-effective care.
- Provide expert clinical support to the home health team, ensuring high-quality care delivery.
- Collaborate with physicians, clinical staff, and other healthcare providers to develop and oversee individualized care plans.
- Ensure accurate and timely completion of OASIS assessments and other required documentation.
- Serve as a key liaison between the clinical team and physicians, fostering effective communication and collaboration.
- Monitor patient progress, intervene when necessary, and ensure care aligns with ACO goals.
- Provide guidance and education to the clinical team on best practices and care management strategies.
- Support the team in meeting regulatory and compliance standards related to Medicare home health and ACO requirements.
- Identify opportunities for process improvement and contribute to quality initiatives.
Qualifications:
- Current and unencumbered Registered Nurse (RN) license in [Insert State].
- A minimum of 2 years of Medicare home health experience.
- Proficiency with OASIS assessments and documentation.
- Strong clinical knowledge and decision-making skills.
- Experience in care coordination and working with ACOs is highly preferred, willing to train the right candidate.
