We are seeking an experienced and dedicated RN ACO Care Manager to join our home health team in our OCALA, FL office. The ideal candidate will have a strong background in Medicare home health, expertise in OASIS, and exceptional skills in clinical support, patient management, and physician communication. This position plays a vital role in supporting our clinical team and managing patients within an Accountable Care Organization (ACO) framework.

Responsibilities :

• Manage and coordinate care for patients within the ACO to achieve optimal clinical outcomes and cost-effective care.
• Provide expert clinical support to the home health team, ensuring high-quality care delivery.
• Collaborate with physicians, clinical staff, and other healthcare providers to develop and oversee individualized care plans.
• Ensure accurate and timely completion of OASIS assessments and other required documentation.
• Serve as a key liaison between the clinical team and physicians, fostering effective communication and collaboration.
• Monitor patient progress, intervene when necessary, and ensure care aligns with ACO goals.
• Provide guidance and education to the clinical team on best practices and care management strategies.
• Support the team in meeting regulatory and compliance standards related to Medicare home health and ACO requirements.
• Identify opportunities for process improvement and contribute to quality initiatives.
