Position Details:

- Salary: $85,000 per year

Qualifications:
- RN License (active and in good standing)
- 1 year of skilled home health experience using OASIS preferred
- Willing to train the right candidate
- Weekend availability required
- Flexible to travel between Charlotte & Sarasota Counties
Responsibilities:
- Provide skilled nursing visits to patients in their homes
- Complete thorough assessments and OASIS documentation
- Develop, implement, and revise care plans as needed
- Educate patients and caregivers on disease processes and self-management
- Collaborate with interdisciplinary teams to ensure quality care
