Hospice Social Worker (MSW) – Full Time

Responsibilities:
- Assess emotional factors related to terminal illness and evaluate patient/family responses to psychosocial interventions
- Support caregivers by assessing their ability to function and identifying needed resources
- Assist the physician and care team in understanding the emotional and mental health challenges that may exacerbate symptoms
- Identify and address risks related to suicide, abuse, neglect, or safety concerns
- Assess cultural and family dynamics to ensure personalized, respectful care
- Provide crisis intervention, short-term counseling, and guidance on advance directives and funeral planning
- Identify and connect patients and families with community and medical resources to support their care at home
- Participate in the development and revision of the patient’s care plan
What We’re Looking For:
✔ Master of Social Work (MSW) from an accredited program
✔ Preferred: 2 years of social work experience in a healthcare setting
✔ Flexibility in scheduling to meet patient needs
