Responsibilities : Assess emotional factors related to terminal illness and evaluate patient / family responses to psychosocial interventions Support caregivers by assessing their ability to function and identifying needed resources Assist the physician and care team in understanding the emotional and mental health challenges that may exacerbate symptoms Identify and address risks related to suicide, abuse, neglect, or safety concerns Assess cultural and family dynamics to ensure personalized, respectful care Provide crisis intervention, short-term counseling, and guidance on advance directives and funeral planning Identify and connect patients and families with community and medical resources to support their care at home Participate in the development and revision of the patient’s care plan What We’re Looking For :
- Master of Social Work (MSW) from an accredited program
- Preferred : 2 years of social work experience in a healthcare setting
