Responsibilities
- Acts as a liaison between hospice and the facility to maintain effective communication
- Upholds a positive image to the public through effective planning, programming, advertising, special events, and care management
- Specializes in professional Nursing care to patients in facilities
- Interacts with patients, families, physicians, and other Health team members
- Reviews admission data as defined by policies and procedures
- Assesses patient status and responds in timely manner
- Implements priorities for Nursing action in logical sequence according to patient’s needs
- Formulates a plan of care by the appropriate selection of Nurse Practice standards
- Supervises the provision of services provided by LPNs based on the plan of care and provides feedback on performance as indicated
- Associate degree in Nursing with current license
- One year practice as a Registered Nurse
- Acute Care, Home Health, or Hospice experience preferred
- Knowledge of Hospice Philosophy of Care
