Haven Health, the leading Home Health, Hospice, and Palliative Care company in Southern California, is currently hiring for an Triage LVN. This position will be open to Licensed Vocational Nurses.

Positions Available (Weekends Required):

• Part-Time
• Full-time
• Evenings and Nights

Responsibilities:

• Process intake data from referral sources
• Coordinate visits and maintain patient schedules
• Verify and establish authorizations for treatment
• Serve as the liaison between patients and staff
• Perform other duties as assigned
• triage and provide telephonic nursing response to patient calls regarding symptom management, medication lists and plans of care.

Requirements

Qualifications:

• Bilingual in English in Spanish required
• Must be an LVN
• Experience in a medical office preferred
• Knowledge of medical terminology
• Able to work independently with minimal supervision
• Exceptional written and verbal communication skills
• Pleasant phone demeanor
